Our doctors are available Monday through Friday from 8:30am to 5:00pm. We will schedule your appointment as promptly as possible. If you have an emergency situation, every attempt will be made to see you that day.

We try to stay on schedule to minimize waiting at the time of your appointment. Referral of emergency patients can cause delays to the schedule, and your understanding and patience are very much appreciated.

Please assist us at the time of your initial visit to the office by providing the following information:

  • Please bring your referral form and x-rays from your dentist or physician.
  • A list of medications you are currently taking.
  • Please bring all dental and medical insurance information and cards with you at the time of your first appointment.
  • Have available the phone numbers & addresses of your current dentist and physician.

All patients under the age of 18 should be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valve or artificial joints, heart murmur requiring premedication, severe diabetes, or hypertension.)

If your referring dentist has taken x-rays, you may request that they be forwarded to us.




Our insurance coordinators deal with many different insurance companies. Some companies offer many different dental and medical plans. Some companies combine dental and medical coverage. This insurance alphabet soup changes policies and guidelines weekly. At times, it is almost impossible to accurately estimate our patients' insurance co-payment. Many insurance companies will not give out fees until after the treatment is completed. Dealing with these companies can be difficult and time consuming. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance be current.

All co-payments are payable when you check in at the front desk.

If a patient comes to us with a problem that they expect to be covered by medical insurance (biopsies, tumors, TMJ, infections, jaw deformities), they must have a referral from their primary care physician. A referral from a dentist is not adequate for medical insurance coverage. Obtaining a medical referral is the patient's responsibility. We cannot obtain the referral for you, and the referral cannot be obtained retroactively. If you do not have a referral, we will be happy to see you on a cash basis, but your medical insurance company will not pay for your treatment.

Medicare pays us directly for your care. You are responsible for any deductible and co-insurance. Dental procedures (extractions) are NOT covered by Medicare. If Medicare denies your procedure, you are responsible for the charges.

As a courtesy, we will file your insurance claims for you. Upon our receipt of an insurance payment, any balance due will be billed to you. If you have deposited an excessive co-payment, it will be refunded to you promptly.

We will send you a monthly statement. Most insurances will respond within four to eight weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
